Patient Forms

In order to make your visit to our practice as efficient and as personalized as possible, we ask that you complete the registration paperwork prior to your appointment.

If you have completed your paperwork prior to your appointment, please arrive at least 10 minutes early to allow time for check in.  If you have not completed your paperwork prior to your appointment, please arrive 30 minutes early to complete paperwork prior to check in.  Please allow ample time to find parking.

Please be advised if you are late by 15 minutes or more we may need to reschedule your appointment.

Please be sure to bring the following items with you:

• Photo ID
• Current Insurance Card
• Referral from your primary care physician
• Your completed registration forms
• All medications that you are currently taking

Insurance and Payment

Our office accepts many types of insurance, and our billing department makes every effort to assess your insurance coverage before your scheduled appointment in order to accurately determine the payment due at the time of service. Co-pays, co-insurance and deductibles are due at the time services are rendered. We accept the following forms of payment: check, cash and most major credit cards. We always check your payment against the “Explanation of benefits” sent subsequently by your insurance company and will refund any overpayment you have made.

If you are unsure of your benefits, please contact your insurance carrier. If you have questions regarding your bill and/or need to make payment arrangements, please contact our billing department during regular business hours, and we will be happy to assist you.
